    When it comes to facial rejuvenation, not every patient needs or desires an actual surgical procedure. Young patients may only need the benefits of the small improvements obtained with fillers and Botox or Laser services. Some patients have an extraordinary fear of surgery or anesthesia. Others are too old or have health problems that exclude them from surgical procedures.

    Non surgical facial rejuvenation refers to all the options available to patients that simply don’t involve surgery. The most popular procedure by far is the use of Botox for fine lines and wrinkles. Dermal fillers or “plumpers” are substances that are injected into the skin in order to “plump” or fill a wrinkle or crease. In larger amounts these fillers are used as “volumizers,” such as when a filler is used for lip, chin, or cheek augmentation.

    Other procedures include Laser treatments, permanent makeup, medical grade skin care products and services, as well as chemical peels.

    The basic tools of non-surgical rejuvenation are botulinum toxin and fillers:

    What is Gynecomastia? When is it Gynecomastia and When is it Normal?

    Gynecomastia (gy-neco-mas-tia) refers to persistent enlargement of the male breast. In today’s vernacular, we call it “man boobs.” It is caused by the benign enlargement of the glandular tissue of the breast, as well as an accumulation of fibro-fatty tissue.  Prominent nipples and “puffy” areolas are often seen with this condition, and it usually affects both breasts, sometimes asymmetrically.

     

    When is it gynecomastia and when is it normal?

    This question is an issue because there is no black and white when it comes to gynecomastia. Some men simply have more fullness than others and male chest shape varies considerably. The following is a list of some observations that I have made after years of treating patients with gynecomastia:

    • All men have some breast tissue behind their nipple-areola complexes.
    • Some males complain of gynecomastia, but they actually “look” perfectly normal.
    • Many men over age 50 develop more “female” like breasts because of aging changes.
    • The definition of gynecomastia refers to excessive development of the breast in a male.
    • Gynecomastia is obvious when I see it (chest area looks round and full like a female breast).
    • You can’t convince a patient who thinks they have gynecomastia that they are “normal.”
    • I suspect that psychological counseling will not “cure” men with gynecomastia.
    • Extremely fit and muscular men can appear to have gynecomastia.
    • Males with minimal evidence of gynecomastia can suffer tremendously while there are others with “female like breasts” that could care less about their appearance.

    Dr. Caridi of Westlake Plastic Surgery created The Breast Place for women who are running around with unsatisfactory breast enhancement results.  It doesn't hurt to learn what is available, what outcome can be expected, and what is will cost you. 

    One issue with an unsatisfactory breast augmentation is implant rippling. Implant rippling refers to the creases that develop in breast implants that are sometimes palpable through the skin. All breast implants will ripple to some degree; saline ripple more than silicone. Submuscular implants ripple less than subglandular implants because the muscle provides an additional layer of implant coverage.

    Rippling is usually seen along the bottom and sides of the breast because the tissue coverage is thinnest in these areas. Rippling between the breasts is less common, but if present, can be quite distressing as it is a highly visible area.

    Traction rippling refers to creases seen on the breast, when a patient changes position and the weight of the implant pulls on the skin (seen when bending over).  When a patient describes that she can “feel the bag,” this means that the tissue coverage on top of the implant is so thin that you are basically feeling and seeing the breast implant under a thin layer of skin only.

    An excessively “round,” or “stuck on,” appearance of an augmented breast is usually the result from an excessively large or overinflated saline implant or from subglandular implant placement. This can also result from an implant contracture, as well as the use of a high profile implant in a very thin patient.

    Treatment options vary, depending on the specific issues (implant size and type, implant location – above or below the muscle, and tissue characteristics, not to mention patient goals). A saline implant can be exchanged for a silicone variety. Relocation of the implant below the muscle can also help. Elimination of a contracture will soften the breast and treat the rippled, unnatural appearance.  ADM, or Acellular Dermal Matrix (human or pig dermis), can also be used in revision breast surgery to minimize some of these issues.
